A 15-20 minute visit regards acute illnesses such as UTI’s, upper respiratory infections, sick visits, allergies, rashes, back sprain/strain, tick bites, etc. Medication if prescribed is sent to a retail pharmacy for same day pickup and is NOT included in the cost. Most medications prescribed during this visit are under $10 (with or without insurance).

A 20-30 minute visit following up on a chronic medical condition or follow up from prior visit. This can entail reviewing lab work results, following up on management of a chronic disease, tailoring health care plans, medication renewals. This does NOT include the cost of your medications unless previously discussed with the provider. Medication prescriptions will be sent for 3-6 months at a time. (Controlled substances will only be sent for 30 days at a time in regulations of NYS guidelines)

Recommended annually- includes a complete head to toe practitioner guided exam, medication reconciliation, a comprehensive blood work panel with review of lab work afterwards, screenings appropriate for age group, and general counseling for health and wellness.

Includes a practitioner start up plan of weight loss with medication assistance with SEMAGLUTIDE, initial lab work prescription, medication shipped directly to the address of your choosing, and subsequent 2 follow up weight in-check appointments. This package includes enough medication for 2 months (8 weeks) and two follow up appointments

This includes 3 visits over a 3 month period including sick visits and/or follow up visits (a combo may be used with any schedule the patient chooses- ie 1 sick visit, 2 follow up, 2 sick visits 1 follow up, 3 sick visits). This does NOT include the cost of any medication that may be prescribed. After 3 months, the membership expires and any unused visits cannot be rolled over or “banked”. This membership cannot be combined with ANY other packages or exams.

This package includes 3 appointments of care over the span of 3 months and includes a blood pressure machine sent to your address, medication and dietary guidance and management, 3 visits (including initial visit), review of blood pressure logs, and lab work required while on blood pressure medications. This does NOT include the cost of blood pressure medications. These appointments are ideally 3-4 weeks apart.
